[Veritas-vx] problem setting up I/O fencing

The disks have to be initialized as Veritas disks before they can be
used in a disk group. Your "vxdisk list" shows them in error state which
is normal for uninitialized disks I think. Try running
/etc/vx/bin/vxdisksetup -fi <disk> and see what "vxdisk list" looks like
afterwards. If the disks are online at that point your vxdg init should
work.
