[Veritas-bu] Duplication from VTL to LTO problem

Our environment, NBU 6.5.4 running on HP-UX 11-31 with a Data Domain VTL
and MSL6060 tape library.   I'm trying to duplicate from a DataDomain
vtl to an lto tape.  I have been duplicating things off to tape for a
while and needed to do it again but am now having issues.  It has worked
in the past but I'm not sure what, if any changes have made since the
last successful duplication happened.   I have watched this a few times,
the tape gets put in the drive and is written to but then gets ejected
before the job is done which causes the error below.  This isn't the
complete log of the job but shows what I'm getting.  I have tried
splitting up the file list of images and again, sometimes it works but
not consistently.  I'm kind of stumped and don't know if this may be a
hardware issue or NBU.  Any suggestions or hints will be appreciated.
